Cricket bone stress injuries

With the cricket season in full swing, it is important for us to understand the potential risk factors and associated injuries with this hugely popular sport here in Australia. With the evolution of the game at the club and female level, as well as the introduction of different forms of the game such as T20, there is no doubt the game is growing rapidly. There are numerous injuries associated with the game both on the fielding and batting sides, ranging from foot and ankle sprains to soft tissue muscle strains and shoulder injuries; both acute and chronic in nature. However, the most prevalent and susceptible group are the fast-paced bowlers. This sub-group commonly suffers from lumbar spine injuries, and bone stress injuries account for the most time missed from game play.

That being said, it is important to understand what bone stress is and how we can effectively prevent and manage these conditions.

Essentially, bone stress related injuries occur as a result of continued overload of our skeletal system and there may be numerous underlying risk factors associated with such a presentation. Ultimately, bone is constantly remodelling, and a correct ratio between bony breakdown and creation helps to maintain appropriate bone health. When this homeostatic process becomes unbalanced, we see the development of bone stress.


The development of bone stress is commonly thought of as a process and not generally associated with a single event. The predisposing factors are described below, but are not limited to the following:

  • Workload or overtraining
  • Prior history of bone stress injuries
  • Faulty body mechanics
  • Change of footwear or surface

The severity of bony stress injuries is situated on a continuum ranging from stress response/reaction and partial or complete fractures. Healing times are also variable and related to the severity of the injury and range from 4 weeks to 16 weeks away from training and games.


Importantly, what can we do to help prevent and manage these injuries to allow our cricket players maximum time spent on the field? Like any sporting injuries, management plans are multifactorial and include modalities such as graduated loading programs, progressive core strengthening and bio-mechanical analysis. Good communication between the medical team and coaching staff is also imperative throughout the rehabilitative process to ensure no set-backs occur and that the athlete has a seamless transition from one stage onto the next.
